This service is especially beneficial for patients recovering after a recent hospital stay or inpatient rehabilitation and is designed to work seamlessly alongside Home Health services.

We also help ensure continuity of care by coordinating with your established primary care provider until you are able to return to their office. While you are receiving Home Health services, our provider can sign Home Health orders when appropriate and collaborate closely with your care team to provide a smooth, coordinated care experience.

When under our care, we know you have already been through a lot. We are here to help you get organized and further navigate your health to get you back on track. We will make specialist referrals if necessary and provide in home services such as blood lab analysis, urine analysis, X-rays and ultrasounds. If DME is required we will order that too! (Hospital bed, walker, bedside commode)
