In Home Primary Care for Patients
In home primary care visits include in person visits with a primary care provider. You can also choose to do follow up visits by telemedicine.
This option works best after a recent hospital stay or in patient rehab stay while also working with Home Health.
Our service includes medication management, general and acute care assessments with a plan of care specific to you.
Our provider can coordinate with your regular pcp until you are able to get back to them. While you work with home health care when it’s appropriate we will sign their orders and coordinate the patient care process for you.

When under our care, we know you have already been through a lot. We are here to help you get organized and further navigate your health to get you back on track. We will make specialist referrals if necessary and provide in home services such as blood lab analysis, urine analysis, X-rays and ultrasounds. If DME is required we will order that too! (Hospital bed, walker, bedside commode)
